Nontuberculous Mycobacterial Empyema in an Immunocompetent Adolescent

Abstract
Nontuberculous mycobacterium (NTM) species are mycobacterial species other than those belonging to the Mycobacterium tuberculosis complex and Mycobacterium leprae. Mycobacterium lentiflavum is one of the slow-growing NTM. M. lentiflavum has recently been described as an emerging human pathogen. Most of the pulmonary diseases caused by M. lentiflavum were detected in immunocompromised patients. There are only a few reports of M. lentiflavum in immunocompetent children causing empyema. In this article, we report a rare case of empyema caused by M. lentiflavum in a 16-year-old previously healthy girl. She came to our hospital due to cough, fever and dyspnea. High resolution computed tomography of the chest demonstrated centrilobular nodules and focal area of ground-glass attenuation in the superior segment of right lower lobe and large amount of pleural effusion in the right lung. NTM was isolated consecutively for three times from the expectorated sputum specimens. All isolates were identified as M. lentiflavum by PCR-restriction fragment length polymorphism (PCRRFLP). Treatment for NTM requires multiple antimycobacterial drug, including isoniazid, rifampicin, ethambutol and macrolide for at least 12 months after negative sputum culture.
